Community Guardianship

Community Guardianship assists seniors who may become incapacitated and require someone to take care of their personal & financial needs.

Community GuardianshipAfter a complete assessment, the Community Guardianship Program creates an all-inclusive care plan to meet the individual needs of each client. The program is committed to assisting seniors maintain their independence. As court-appointed legal guardians for "at risk" adults, we provide appropriate supports such as medical and psychiatric care, financial and legal aid as well as home maintenance assistance. For those who receive 24-hour care in a facility, we maintain a relationship with staff to ensure proper care is being provided. Our "care workers" provide continuity of care through the last stages of life.

The Community Guardianship Program staff work with other Family and Children's Association programs to provide the highest quality of care for our clients. These programs include the Ombudservice program which provides facility volunteers; Financial Counseling and CHEC which provide reverse mortgages; HIICAP which provides health insurance information; and the Long Island Counseling Center which provides mental health services.

We provide complete care management:

  • Needs assessment and plan development.
  • Assistance in arranging home care and medical care.
  • Long-term care assessment and planning.
  • Money management including banking, payment of bills, preparation of tax returns, processing insurance claims.
  • Assistance with public entitlements - Medicare, Medicaid.
  • Counseling and consultation.
  • Ongoing case evaluation.
  • Regular visits.
